I consent to the following: Medical Authorization: My child has permission to attend the Midwest Select Volleyball Evaluations. I have no knowledge of any physical impairment that would affect or be affected by my child’s participation. In the event of an emergency in which my child requires medical care, I authorize the staff of the camp to act for me and to obtain for them whatever medical treatment the staff, in its best judgment deems necessary and appropriate. In the event that medical treatment is deemed necessary, I give my permission, so that my child may receive medical treatment. I agree to hold the Midwest Select and their staff harmless for any liability arising out of good faith action involved in the camp. Media Release: This form allows consent form authorizes Midwest Select Volleyball to use and print photographs and any other form of media material for educational, informational and promotional purposes. Images may be used, but is not limited to, social media and newsletters. I agree to hold The Midwest Select Sports Club and The Battle Creek Y Center and their staff harmless of any liability arising out of good faith action for treatment of a medical emergency or any injury incurred during any Midwest Select events. This includes practices, tournaments, clinics, lessons and team events.
