U13 must be born on or after July 1 2011 cannot turn 14 before July 1,2024

U14 must be born on or after July 1 2010 cannot turn 15 before July 1,2024

U15 must be born on or after July 1 2009 cannot turn 16 before July 1,2024

U16 must be born on or after July 1 2008 cannot turn 17 before July 1,2024

U17 must be born on or after July 1 2007 cannot turn 18 before July 1,2024

Team Placement Process

I understand the team placement process: An announcement will be made at tryouts regarding when to expect team placements to be posted. Athletes will have until the posted time to accept their team offer. Select the commit/decline button on the MWS Team Announcement page (Volleyball>Tryout Information>Team Announcement) to accept your spot on the team. If a response is not received by the posted time, the spot will be offered to the next athlete on the list. Please note, the first payment is due at the time of commitment. How do you know if your athlete was selected for a team? Each athlete was given a number at tryouts. Tryout numbers are what is listed in the team listings. Check the MWS website and MWS Facebook page for your athlete's number. Please visit the volleyball homepage for more details on team fees and scheduling.

Medical Authorization

I consent to the following: Medical Authorization: My child has permission to attend the Midwest Select Volleyball Evaluations. I have no knowledge of any physical impairment that would affect or be affected by my child’s participation. In the event of an emergency in which my child requires medical care, I authorize the staff of the camp to act for me and to obtain for them whatever medical treatment the staff, in its best judgment deems necessary and appropriate. In the event that medical treatment is deemed necessary, I give my permission, so that my child may receive medical treatment. I agree to hold the Midwest Select and their staff harmless for any liability arising out of good faith action involved in the camp. Media Release: This form allows consent form authorizes Midwest Select Volleyball to use and print photographs and any other form of media material for educational, informational and promotional purposes. Images may be used, but is not limited to, social media and newsletters. I agree to hold The Midwest Select Sports Club and The Battle Creek Y Center and their staff harmless of any liability arising out of good faith action for treatment of a medical emergency or any injury incurred during any Midwest Select events. This includes practices, tournaments, clinics, lessons and team events.
